Tutkimussuunnitelma:

In recent years the incidence and mortality of CDAD has increased worldwide. The objective of the study is to determine the clinical characteristics of patients with extraintestinal Clostridium difficile infection. All deep bacterial cultures of Clostridium difficile (approximately 20 patients) since 1990 in HUCH will be evaluated retrospectively. Two bacteremia will be described and literature reviewed.

Helsinki region experience with Clostridium difficile ribotype O 27/ other strains: mortality and severity of the disease and risk for relapse

There is no funding source of this study at the moment ( 2.5.08)

The primary sponsor is Helsinki University Central Hospital, Department of Medicine, Division of Infectious Diseases

Study type: retrospective epidemiological study

Purpose:To compare severity, 30-day mortality and relapse of C. difficile disease with 027 virulence factors versus C difficile disease without these virulence factors. Patients will be stratified by McCabe and Jackson for severity of underlying illness. Severe C. difficile disease is defined as having one or more of shock, paralytic ileus, pseudomembranous colitis, toxic megacolon or peripheral leukocyte count > 15 000 cells/mL.

Diagnosis and main criteria for inclusion (target population):
Male and female CDAD subjects, age 18 years and above with C difficile infection in Helsiki region hospitals ( population 1.5 million ) 11/07-5/08

Tutkimussuunnitelma:

Influence of a new polycationic disinfectant on Clostridium difficile incidence and environmental colonisation

The study is funded by TEKES ( Finnish Funding Agency for Technology and Innovation ) and Softcare Ltd. The primary sponsor is Helsinki University Central Hospital, Department of Medicine, Division of Infectious Diseases.

The aim of this study is to 1) test the efficacy of PHMG-based disinfectant against C. difficile spores, 2) test whether it reduces the incidence of C. difficile associated disease (CDAD) and 3) evaluate cost.

New disinfectant will be introduced in three wards for hand hygiene and environmental disinfection in CDAD patients� rooms. It replaces alcohol based disinfectant in study wards. Three wards will be control wards and continue using alcohol based hand disinfectants and routine environmental cleaning and disinfection with quats. The rooms for showers and toilets will be coated with biocide coating as well as bed frames.After 6 month�s intervention period, the incidence of CDAD cases after will be compared to that during the preceeding 6 months. Surveillance for environmental and HCWsŽ hand contamination by C. difficile will be performed by taking microbiological samples both from environmental sites and hands twice before intervention and then twice in month within intervention period.
C. difficile cases will be searched by active laboratory based surveillance. Each case will be confirmed both by microbiological tests and clinically by infectious diseases consultant (Eero Mattila) in all study wards. Patient records will be studied for extra costs caused by infection. Prolongation of hospitalization will be examinated by AEP (appropriateness evaluation protocol) method by infectious diseases consultant (Eero Mattila) in Meilahti wards. Cost of extra day is the average patient day cost in HYKS. ./..

Unit costs of extra procedures and medication caused by C. difficile infection will be examinated retrospectively from patients records. Medication costs include both antibiotics to treat C. difficile and medication used to complications due to infection. Also medications prescribed to home use will be calculated. Isolation cost will be included (if the patients are not isolated as recommended, isolation costs will be approximated). Rehospitalization and visits to policlinics will be counted if the reason for them is C. difficile infection. Medication costs to patient will be counted but not the opportunity costs for time.
